Checklist
Who will own the policy? This is the person that can make changes to the policy after the policy is written. This person can be the same person that is insured.
- Their Name
- Their address
- Social security or tax id number?
Who will be insured? If this person passes away, a life insurance claim is made.
- Their Name
- Their address
- Social security number
- What state and city were they born in
- What medicines are being taken?
- Name, Address, Phone number to the Doctor this person sees on a regular basis.
- Are there other life insurance policies in place for this person? Carrier, policy, number, type.
- Is the insured a citizen of the US? If not, please provide Green Card Information.
Who will be the beneficiary or beneficiaries in the event of the death of the insured? Many times there is a primary beneficiary and then alternate beneficiaries in the event the primary beneficiary is not alive at the time of a claim. For each beneficiary, the following information is needed:
- The beneficiaries name(s)
- Alternate beneficiaries can be defined with % of proceeds to go to each. What % needs to be defined for each beneficiary.
- address
- birthdate and place
- social security number
If their is an additional insured to be included such as a spouse or business partner, then list the same information for this “Insured” and the respective “Beneficiary name(s)” too.
Children can also be added to the policy at a very low cost. Adding childern to the policy is as low as $5.00 to $20.00 and and include one or more children at the same price. To add coverage for a child or children be ready to provide:
- Name
- Birth date
- Doctor’s name, address, phone number
- Social Security number
- How related
